Patient Registration Instructions
Patients now have 24/7 access to their confidential information from our secure patient portal. This portal access is secure and meets all compliance requirements outlined by the Health Insurance Portability and Accountability Act (HIPAA) of 1996.
We are now using electronic medical records and are able to offer you the convenience of entering your demographics and medical history information online prior to appointment, and at your leisure. You may also view a Summary of prior office visits online. Here are the sample steps to begin:
Step 1: As a new user click the FIRST TIME USER option to access the patient portal site.
Step 2: Enter your Last Name and the one-time access code provided to you by our phone room staff. If you do not have a code, please call (888) 970-7546 to request your code during normal business hours. Once entered, you will be redirected to a new page requesting you to create your own unique username and password (username must contain at least one number). Once you create a username and password, the one-time access code is no longer valid.
Step 3: Click on the Demographics link located at the top of the page to review and edit your contact information. Please verify that everything has been entered correctly and then click the “Save” button.
Step 4: Click on the Forms link to be taken to the online Medical History Form. Click on the form listed to open it and begin filling in each section, using the “next” button to access the next section. If you have to stop before completing the form, click on “Save and Resume Later” to save your changes, otherwise select “Submit” when you are done. Once submitted, your Medical History Form and no longer be available for editing or viewing online and will be in your medical record on the day of your appointment. Remember to keep your username and password available to use as a RETURNING USER.
Step 5: Click “log out” when you are done.
